INSTRUCTIONS FOR REGISTRATION:

1. Complete the entire registration form and upload a photo of your Camper.
   
2. Click “Next” to continue to the registration payment page (PayPal).

Pay the one-time, non-refundable Summer Camp Registration and Activities Fee to complete initial registration.

CAMP FEES (NON-REFUNDABLE):
$65 One-time Registration and Summer Camp Activity Fee to be paid upon submission of this form.
$200:  Each week of Day Camp ($100 if 100 Parent Involvement Credits are earned)
$250:  No Kids Allowed Week (Week 3)
$250:  Residential Week at Camp Twin Lakes (Week 5)
Please note:  If your camper does not attend a week of camp that they are registered for without advance notice, responsibility for payment will be required.

SUMMER CAMP 2012 WEEKS:
Week 1: June 4-8
Week 2: June 11-15
Week 3: June 18-22 (No Kids Allowed, 16+)
Week 4: June 25-29
Week 5: July 8-13 (Residential Week @ Camp Twin Lakes)
Week 6: July 16-20
Week 7: July 23-27 (Field Trip Week)    

CAMP INFORMATION:

1. Camp is open to all individuals with a diagnosed developmental disability, starting at age 4.
   
2. Camp is held Monday through Friday from 9:00am – 3:00 pm.  Lunch must be brought from home.
   
3. Parents/guardians are responsible for transportation to and from camp.  Bus service is available through some school systems.
   
4. Morning activities are held at ESP.  Afternoon activities include bowling on Monday and swimming Tuesday – Friday.

FINANCIAL AID:

1. Parent Involvement Credits:  Parents may earn credits throughout the year to reduce camp fees by participating in ESP fundraisers and events.  To be considered for the $100 reduction in camp fees, completed credit forms must be turned in to ESP by April 15, 2012.
2. Scholarships:  Limited, partial scholarships are available to families in need. To apply for scholarship consideration, send a brief summary of your financial situation to janet@extraspecialpeople.com by April 15, 2011.  All scholarship requests are confidential.

MANDATORY REGISTRATION DAY: May 5, 2012
For Parents, Campers, ESP Counselors and Staff.  Turn in camp registration forms, meet with leadership staff for your child’s unit, discuss your child’s needs, and meet summer staff.
